Visual performance for speech-language pathology

Visual performance shows up in your caseload as a confound, not as a referral question. A student who loses the line, re-reads, or fatigues after four minutes of print will look like a comprehension problem on a language measure and like an attention problem to a teacher. This supplement separates what visual performance contributes from what language contributes, so your data describes the skill you meant to sample.
Visual load in language sampling and testing
Any task you deliver in print carries a visual performance load that your scoring rubric does not control for. When the stimulus is read rather than heard, a visual efficiency limitation lowers the score without touching the language system.
- Compare listening comprehension with reading comprehension on parallel content — a wide split with intact listening points away from language.
- Watch the first minute against the fourth: language performance is stable across a passage, visual efficiency decays.
- Note head turning, finger tracking, loss of place, and re-reading as behaviors, not as effort or motivation.
- When you must use print, present one enlarged, high-contrast item at a time to remove the efficiency variable.
Decoding, comprehension, and visual efficiency are three different failures
Phonological decoding weakness, language comprehension weakness, and visual efficiency weakness produce overlapping classroom complaints and distinct patterns on task.
- Decoding: errors are phonologically patterned, and accuracy does not improve when the visual field is simplified.
- Comprehension: accurate oral reading with poor retell; the pattern holds in the listening modality too.
- Visual efficiency: accuracy is intact briefly, then rate and place-keeping degrade; performance improves markedly with a line guide, larger print, or single-line presentation.
- These co-occur often. Naming which one you are treating is what keeps a plan defensible.
AAC displays and array design
Array size, symbol spacing, contrast, and field placement are visual performance decisions that determine whether a communication system is usable.
- Start with fewer, larger, higher-contrast cells and expand only after selection accuracy is stable across a full session.
- Check for consistent neglect of one region of the display — that is a visual finding to refer, not a learning problem.
- Keep background clutter and glare off the display surface; a laminated page under fluorescent light can cost you accuracy.
- Position the display for a consistent, comfortable gaze angle rather than centering it out of habit.
Working alongside the OT
The most useful thing you can hand an OT is a description of visual performance behavior tied to an occupation, with the time course attached.
- Report the task, the duration to breakdown, and what changed the performance — that maps directly onto the levels.
- Ask for the VPA™ profile and the level that is being treated first, so your session accommodations match it.
- Co-write the accommodation language once so the IEP does not carry two different versions of it.
Refer, do not work around
- · Any report of double vision, or an eye that turns or drifts
- · Head tilt or consistent head turn to see
- · Reading or near-work headaches, eye pain, or nausea
- · A change in visual behavior after an illness, injury, or concussion
- · Consistent inattention to one side of the page, display, or room
- · A student who has never had a full eye examination, not just a school screening
Scope boundary. Assessment and treatment of visual performance deficits, including administration and scoring of the VPA™, remain within occupational therapy scope. What this supplement gives you is language for what you are seeing, decision rules for when to refer, and modifications you can make inside your own sessions so visual load stops confounding the skill you are actually treating.
